CKD in the Primary Care Setting: Implementing Strategies into Practice

Register for this three-part program which will address chronic kidney disease (CKD) testing and evaluation in primary care, diabetes and CKD management in primary care, and blood pressure and CKD management in primary care. Faculty will discuss co-morbidities that increase a patient's risk of CKD, and strategies to reduce risk and improve outcomes. Implementation of CKD care into the primary care setting will also be addressed.

  • Describe management of cardiovascular health in people with high blood pressure and CKD
