Session 5 in the Navigating Transplant: A Guide to Empowering Your Patients series, we welcome two dialysis social workers as they share their clinical experiences. Through case presentation and discussion, we will focus in on the challenges and opportunities that face social workers and their patients during the transplant education, referral, and work up stages of the journey. Equally important, through these examples, we will share strategies and resources that will help you address barriers and empower your patients and their care partners forward through the transplant process. This virtual session will be on May 22, 2024 at 12pm PT / 2pm CT / 3pm ET and is not for CE credit.
