Kidney Care Management Program Coordinator, Dialysis Center of Lincoln


Michelle L. Johnson, PhD, RN is the Kidney Care Management (KCM) program coordinator at Dialysis Center of Lincoln and is also a nurse educator within the program. Michelle received her nursing diploma from Bryan Memorial Hospital, School of Nursing, RN-BSN from the University of Nebraska Medical Center (UNMC) College of Nursing, MSN from Nebraska Wesleyan University, and PhD from UNMC College of Nursing. Michelle has spent most of her nursing career in nephrology nursing and in academia teaching all levels of nursing students. She has come full circle with her current position teaching and supporting individuals living with CKD.

 Michelle works closely with her interdisciplinary colleagues, a renal dietitian and renal social worker, to individualize the education and support they provide KCM participants. An individualized approach is vital to meeting each participant’s needs and teach them how to slow or abate kidney disease progression. The novel KCM program collaborates with the local nephrology clinic and is the only one of its kind in Lincoln, Nebraska. The program began in December 2023 and has expanded into the community since its inception. The KCM program is raising CKD awareness through community-based engagement and collaborative community partnerships.
