A 60-minute case-based learning experience to empower dialysis facility staff to improve home dialysis (PD and HHD) initiation and retention.

Nationally recognized home dialysis care team experts will address the spectrum of challenges (infrastructure, clinical, psychosocial) associated with growing a home dialysis program.

The entire dialysis facility is encouraged to join these case-based interprofessional sessions. Only one registration is required per facility.

Target Audience

All healthcare professionals involved in the care of home dialysis patients and/or involved in educating patients on dialysis modality selection.

  • How to establish a culture of promoting home dialysis
  • Modality Education with emphasis on home dialysis (includes peer mentoring)
  • Patient/family/home assessment by the interprofessional team
  • Best practice training techniques for successful home dialysis
  • Involving the family/support people in training and follow up
  • Ingredients for a successful home dialysis team
  • Transitional unit
  • Technology
  • Improvement of technique survival

  • Assumptions about barriers
  • Patient decision support
  • Infection prevention
  • Psychosocial adjustment/adapting
  • Clinical strategies to help patients feel comfortable performing dialysis at home
  • PD catheter placement/growing relationship with PD catheter surgeons
  • Non-infectious complications in home dialysis
  • Home hemodialysis prescription using various platforms
  • Troubleshooting the PD prescription
  • Diet (for both PD and home HD)
  • Supporting patients to fulfill life while on dialysis
  • Logistics (delivery/storage of supplies)
