Please complete the following information to include a tribute to deceased donors (and their families) on the U.S. Transplant Games website. These tributes will ONLY be posted on the U.S. Transplant Games website. (Donor families and family/friends of living donors will also have an opportunity to share a tribute for the Souvenir Program during the online registration process.)
